Salbutamol (INN) or albuterol (USAN) is a short-acting β2-adrenergic receptor agonist used for the relief of bronchospasm in conditions such as asthma and COPD.
Salbutamol sulphate is usually given by the inhaled route for direct effect on bronchial smooth muscle. This is usually achieved through a metered dose inhaler (MDI), nebuliser or other proprietary delivery devices (e.g. Rotahaler or Autohaler). In these forms of delivery, the effect of Salbutamol can take place within 5 to 20 minutes of dosing. Salbutamol can also be given orally or intravenously. However, some asthmatics may not respond to these medications as they will not have the required DNA base sequence in a specific gene.
Salbutamol became available in the United Kingdom in 1969 and in the United States in 1980 under the trade name Ventolin. Salbutamol is specifically indicated in the following conditions:
* acute asthma * symptom relief during maintenance therapy of asthma and other conditions with reversible airways obstruction (including COPD) * protection against exercise-induced asthma * hyperkalaemia, especially in patients with renal failure * can be aerosolized with a nebulizer for patients with Cystic Fibrosis, along with ipratropium bromide and pulmozyme.
As a β2-agonist, salbutamol also finds use in obstetrics. Intravenous salbutamol can be used as a tocolytic to relax the uterine smooth muscle to delay premature labour. Whilst preferred over agents such as atosiban and ritodrine, its role has largely been replaced by the calcium-channel blocker nifedipine which is more effective, better tolerated and orally administered. As with other β2-adrenergic receptor agonists, salbutamol binds to β2-adrenergic receptors with a higher affinity than β1-receptors. In the airway, activation of β2-receptors results in relaxation of bronchial smooth muscle resulting in a widening of the airway (bronchodilation). Inhaled salbutamol sulfate has a rapid onset of action, providing relief within 5-15 minutes of administration.
In tocolysis, the activation of β2-receptors results in relaxation of uterine smooth muscle, thus delaying labour.

While salbutamol is well-tolerated, particularly when compared with previous therapies such as theophylline, like all medications there exists the potential for adverse drug reactions to occur - especially when in high doses, or when taken orally or intravenously.
Common adverse effects include: tremor, palpitations and headache. (Rossi, 2004)
Infrequent adverse effects include: tachycardia, muscle cramps, agitation, hypokalemia, hyperactivity in children, and insomnia. [1]
The (S) enantiomer of salbutamol can inhibit the anti-inflammatory effect of steroids prescribed to treat asthma. However, the (R) enantiomer stimulates the steroid's effect and the overall effect of the two isomers is unclear[3].

Levalbuterol, the R-enantiomer of salbutamol, is sold as Xopenex.

U.S. regulators have announced that albuterol inhalers containing chlorofluorocarbons (CFCs) will be banned in the United States beginning in 2009. This type of asthma inhaler had previously been given "essential use" status, exempting it from a national CFC-production ban. However, GlaxoSmithKline, Ivax Corp., and other manufacturers are expected to produce adequate supplies of alternative inhalers by 2009 and will offer discounts for those who cannot afford the newer versions, which cost about $20 more.

Because the effects of salmeterol and formoterol usually last about 12 hours, doses should never be taken more than two times a day or less than 12 hours apart. Salmeterol and formoterol are not substitutes for your oral or inhaled corticosteroid medicine. If you begin taking salmeterol or formoterol, the dosage of your corticosteroid medicine should not be changed or stopped even if you begin to feel better. If you have any questions about this, check with your doctor. Some epinephrine preparations are available without a doctor's prescription. However, do not use this medicine unless you are seeing a doctor about asthma. Do not use this medicine if you have been hospitalized for asthma treatment or if you are taking a prescription medicine for asthma, unless you have been told to do so by a doctor. When you use the inhaler for the first time, or if you have not used it in a while, the inhaler may not deliver the right amount of medicine with the first puff. Therefore, before using the inhaler, you may have to test or prime it. To test or prime most inhalers : Insert the medicine container (canister) firmly into the clean mouthpiece according to the manufacturer's directions. Check to make sure it is placed properly into the mouthpiece. Take the cap off the mouthpiece and shake the inhaler three or four times. Hold the inhaler well away from you at arm's length and press the top of the canister, spraying the medicine into the air two times. The inhaler will now be ready to provide the right amount of medicine when you use it.
To use most inhalers : - Using your thumb and one or two fingers, hold the inhaler upright, with the mouthpiece end down and pointing toward you.
- Take the cap off the mouthpiece. Check the mouthpiece to make sure it is clear. Then, gently shake the inhaler three or four times.
- Breathe out slowly to the end of a normal breath.
- Use the inhalation method recommended by your doctor:
- Open-mouth method—Place the mouthpiece about 1 to 2 inches (2 finger widths) in front of your widely opened mouth. Make sure the inhaler is aimed into your mouth so the spray does not hit the roof of your mouth or your tongue.
- Closed-mouth method—Place the mouthpiece in your mouth between your teeth and over your tongue with your lips closed tightly around it. Make sure your tongue or teeth are not blocking the opening.
- Start to breathe in slowly through your mouth. At the same time, press the top of the canister one time to get 1 puff of medicine. Continue to breathe in slowly for 3 to 5 seconds. Count the seconds while breathing in. It is important to press the canister and breathe in slowly at the same time so the medicine gets into your lungs. This step may be difficult at first. If you are using the closed-mouth method and you see a fine mist coming from your mouth or nose, the inhaler is not being used correctly.
- Hold your breath as long as you can up to 10 seconds. This gives the medicine time to settle into your airways and lungs.
- Take the mouthpiece away from your mouth and breathe out slowly.
- If your doctor has told you to inhale more than 1 puff of medicine at each dose, gently shake the inhaler again and take the next puff following exactly the same steps you used for the first puff. Press the canister one time for each puff of medicine.
- When you are done, wipe off the mouthpiece and replace the cap.

This paper reviews the published toxicology of salbutamol. Salbutamol is a relatively selective beta 2-adrenoreceptor stimulant with rapid, potent bronchodilator activity and only minor inotropic or chronotropic effects. It was not found to be mutagenic. LD50 values and other acute studies indicated low toxicity. Findings published for repeat dose studies were mainly uneventful. Tachycardia and flushing of the skin were observed in dogs. There were several findings peculiar to the rat--growth of the salivary gland, enlargement of the Harderian gland, an increase in colloid in the pituitary, and mesovarian leiomyomas. Increases in heart weights associated with inflammation, hypertrophy of muscle fibres, focal myocardial necrosis and fibrosis were seen in rats. Malformation, in the form of cleft palate, was reported in mice but not in rats or rabbits. These treatment related effects reported for salbutamol are not compound-related but rather are class-related. I have been Asthmatic for several years a bout of bronchitis a few years ago caused this. My doctor has being trying different inhalers with me as the old CFC propelled ones are going out. I was on this one and of the CFC free ones it was the closest in similarity to my trusty old ventolin inhaler, the only problem being that this is not an on demand inhaler. What do I mean by that the old style inhalers which this is one of required you to time the puff with a deep breath, which was quite a skill in itself and fairly inefficient. The new on-demand inhalers only puff when activated by your breath and penetrate deeper into your lungs which is a good thing. Now if you are an asthma sufferer then your doctor should be trying to help you find a suitable on-demand inhaler that suits you. However this can take some time I have tried 3 different types and the one that was most suitable has since been taken off the market (Boo Hoo). I have stressed to my Doctor that when I am trying a new inhaler, I should also have one that I have confidence in. There is nothing worse that having to use an inhaler that you are not sure of during an asthma attack as during that time you are not really in a logical frame of mind, and using something that is familiar is always better as it helps you relax.
